Clinical Specialist Pharmacist – Emergency Department – Full‑Time, Day Shift
Department: Inpatient Pharmacy Services
Schedule: Full‑Time, Day Shift (Every 5th Weekend)
Reports To: Pharmacy Clinical Team Leader, Pharmacy Manager
Setting: Acute‑Care Hospital
Position Summary
The Clinical Specialist Pharmacist provides advanced, patient‑centered pharmaceutical care in an advanced hospital environment. This role is responsible for optimizing medication therapy, and ensuring safe, evidence‑based practice through established automatic renal dosing and anticoagulation adjustment protocols. The pharmacist collaborates closely with physicians, nurses, and allied health professionals to improve clinical outcomes and support the hospital’s mission of high‑quality patient care.
Key Responsibilities
- Collaborate with emergency department (ED) healthcare team to promote medication use that is evidence-based and aligned with national quality indicators to provide patient and family centered care
- Serve as the primary source of drug information for all practitioners and patients within the ED
- Prospectively review and prioritize patient involvement upon arrival based on acuity, condition, chart review, and medication orders.
- Provide prospective medication therapy recommendations based upon patient assessment
- Facilitate medication stewardship (e.g. opioid, antimicrobial, anticoagulation)
- Participate in the bedside management of medical emergencies with a focus on drug selection, preparation, procurement, documentation and history
- Medical emergencies include level II traumas, code resuscitation, stroke code, rapid sequence intubation, sepsis and any other unstable patient within the ED
- Participates in hospital-wide code blue/stroke code response as assigned
- Serves and participates as a pharmacy representative for hospital committees involving code or emergency medical response.
- Assist team as a liaison with the ED team and Poison Control Center in patient care as necessary
- Participate in ED patient care rounds as appropriate
- Periodically evaluate, assess, and adjust ED department Omnicell medication inventories for optimization in collaboration with departmental leadership
- Screen and verify medication orders in the ED for appropriateness and monitor for efficacy. Upon review of a patient’s condition and current orders, recommend additions or modifications to drug therapy as necessary
- Support medication reconciliation technician/pharmacist with focused medication histories, when needed, to determine appropriate ongoing care for ED patients
- Assist with providing proper handoff of ED patients to the unit-based pharmacist where indicated as patients are admitted to an inpatient unit
- Counsel on new and/or continued medications when consulted or when deemed appropriate by the pharmacist
- During low ED census, supports departmental teammates with other operational and clinical duties.
Schedule & Work Expectations
- Day shift with consistent daytime hours.
- Every 5th weekend rotation.
- Holiday rotation as assigned.
- Occasional cross‑coverage for other clinical areas depending on departmental needs.
Qualifications
Required
- Doctor of Pharmacy (PharmD) from an ACPE‑accredited institution.
- Active pharmacist license (or eligibility) by the Alabama State Board of Pharmacy.
- Strong clinical knowledge base and ability to function independently in a fast‑paced environment.
- Proficiency with electronic health records and clinical decision support tools.
Preferred
- Completion of an ASHP‑accredited PGY1 or PGY2 Pharmacy Residency in Emergency Medicine or Critical Care.
- Board certification (e.g., BCPS) is highly desirable.
- Prior experience in a large hospital or health‑system pharmacy.
