Schedule: Full‑Time, 3rd Shift (Overnight)
Department: Inpatient Pharmacy
Reports To: Pharmacy Operations Manager / Director of Pharmacy
The Clinical Pharmacist provides advanced, patient‑centered pharmaceutical care during overnight hours in a large, high‑acuity hospital environment. This position ensures safe, effective, and timely medication therapy through clinical monitoring, order verification, sterile product oversight, and interdisciplinary collaboration. The pharmacist functions as the primary medication expert for all inpatient units during the third shift and supports emergency response activities throughout the hospital.
Perform comprehensive medication order verification for accuracy, safety, and clinical appropriateness.
Provide advanced clinical services, including:
Pharmacokinetic dosing and monitoring (e.g., vancomycin, aminoglycosides).
Renal and hepatic dose adjustments.
Total parenteral nutrition (TPN) evaluation and support.
Anticoagulation management and therapeutic drug monitoring.
Identify and resolve drug‑drug, drug‑disease, and drug‑food interactions.
Document all clinical interventions in the electronic health record (EHR).
Participate in hospital emergency responses (e.g., Code Blue, Stroke Alerts, Rapid Response).
Serve as a clinical resource to physicians, nurses, and other healthcare professionals.
Oversee safe and efficient medication distribution during overnight hours.
Supervise pharmacy technicians and ensure compliance with USP <797>/<800> sterile compounding standards.
Review, prepare, and check sterile and non‑sterile compounded products.
Support automated dispensing cabinet (ADC) management and troubleshooting.
Maintain accurate controlled substance handling and documentation.
Bachelor of Pharmacy or Doctor of Pharmacy (PharmD) from an ACPE‑accredited school.
Active pharmacist license (or eligibility) in the state of practice.
Strong communication skills and ability to work independently in a fast‑paced environment.
Proficiency with electronic health records and pharmacy information systems.
Completion of an ASHP‑accredited PGY1 Pharmacy Residency.
Prior experience in a large hospital or health‑system pharmacy.
Board certification (BCPS or equivalent) is a plus.
True third‑shift position with consistent overnight hours.
